Vaccination against Covid-19 is on Thursday (28), from 8 a.m. to 4 p.m., with a “generic version” strategy of applying the first dose to people 12 years of age and older, as well as a second and third dose to individuals 18 years of age or older. More, no matter you are based in Salvador. The only requirement is that the SUS card be linked to a municipality in the state of Bahia.

The interested party must provide, compulsorily, original and photocopies of the vaccination card, the updated National Digital Vaccination Card (ConectSUS), an identification document with a photo and proof of residence in the municipality of Bahia State.

Other groups not included in the “Liberou Geral” strategy must reside in Salvador and have their name listed on the Municipal Health Administration (SMS) website. It also follows the vaccination of children aged 5 to 11 years who are included in the list of SMS sites, in educational institutions in the capital of Bahia.

Documents:

Child or teenager accompanied by the father or mother: it is necessary to have a name on the SMS site and, at the time of vaccination, to provide the original and a copy of the identity document with the photo of the father or mother present, the original and a copy of the ID of the child / teenager, the originals of the vaccination booklet and the SUS card from Salvador for the child / Teenager.

Child or adolescent unaccompanied by a father or mother: It is necessary that his name be on the website and, at the time of vaccination, must be accompanied by another person over 18 years of age. In addition, a completed immunization form signed by the child’s father (father or mother), must be submitted, together with a copy of the identity document with a photo of the person responsible for signing the document, as well as the original and a copy of the identity document of the child / teenager, in addition To the originals of the Child/Adolescent Vaccination Handbook and SUS Card from Salvador. The vaccine form is available to print on the SMS website.

Adults: Must provide updated ConectSUS and official identification document with photo at the time of vaccination.

Research and Study Volunteers: For the third dose, the Research and Study Volunteers must submit the documents listed above, as well as a statement from the research institute indicating that they have been cleared of the third dose, without prejudice to the continuation of the study/research.

Pregnant women and postpartum women: Pregnant women must provide a printed copy of the prescription. The maternity woman must, in addition to the prescription, provide a copy of the child’s birth certificate or acknowledgment of live births.

Immunosuppressants – 4th dose and elderly people aged 80 years or older: Interested persons must be 18 years of age or older, and have already taken the three doses from their vaccination schedule, having taken the third dose by the date specified in the strategy. In addition, it is necessary to have a name on the SMS site.

Exceptional cases of failure to provide complete documentation will be dealt with individually at the time of vaccination itself, as has been the case since the beginning of the strategy.
